Sports physical form
A sports physical (pre-participation evaluation) form has two halves: a history section a parent fills out about the athlete's health background, and a clinical exam section only a physician can complete and sign. Schools and leagues typically require one before each season.
Who fills it out
A parent or guardian completes the history half; a licensed physician, nurse practitioner, or physician assistant completes and signs the exam half.
