School registration form
A school registration (or new-student enrollment) form is what a school or district asks a parent or guardian to complete before a child starts or transfers. It's typically the first paperwork of the school year, and often the longest — usually delivered as a PDF packet, a Word document, or a portal web form, depending on the district.
