School emergency contact form
A school emergency contact form (sometimes called an emergency card) lists who a school should call, and in what order, if something happens to a student and a parent can't be reached. Schools typically ask for it every year, and again for field trips or after-school programs.
Who fills it out
A parent or legal guardian, once per child, usually at the start of each school year and again for any activity that requires its own contact list.
What this kind of form usually asks for
This is a general description of the category — not any specific school, camp, landlord, or provider's actual document. The exact fields on the form in front of you may differ.
- Student name and classroom or teacher
- Parent/guardian contact numbers, in priority order
- Backup contacts authorized to pick the student up
- Known allergies or medical conditions the school should know about
- Primary physician name and phone number
What FillOnce can do
- Fill a PDF or Word emergency contact card from contacts already saved in your vault
- Keep the same contact list consistent across every child's form instead of retyping it each time
What it can't do
- Submit the card to the school or update it in a school's own records system
- Decide contact priority order for you — that's a judgment call only you can make
