FillOnce
Fill patient intake PDFs
Same med list. New clinic PDF.
Fill it without retyping.
Allergies, medications, surgeries, insurance, emergency contacts — stored once, encrypted. Fill structured patient intake PDFs or Word packets when we detect fields, then review before you submit. Manage kids and aging parents as separate profiles.
Why FillOnce
Built for medical intake
Medications, allergies, family history, past surgeries, the long lists every new specialist demands. Stored once. Reused on the next structured intake when fields match.
Manage everyone you care for
Your kids' immunization records, your aging parents' med list, your own history. Each in their own profile, each filled separately.
Private by design
With your zero-knowledge vault set up, saved values are encrypted with a key only you hold, we can't read them. To match a form we do see its field labels, and an import passes that file through us to pull the values out. Matching never goes to an outside AI, and your medical data isn’t training anyone’s model.
The intake forms that ruin waiting rooms
New patient packets
Medical history, current meds, allergies, family history, the long first-visit clipboard, filled from your vault in a couple of clicks.
Specialist referrals
Cardiologist, neurologist, GI, every new specialist wants the same intake info. Stored once, ready always.
Pharmacy & infusion centers
Med lists, dosing schedules, allergies, pre-filled before the technician asks.
Dental & vision
Insurance, last X-ray date, allergies to anesthesia, for the whole household.
How it works
Save details from a form you’ve filled
Upload a filled PDF, Word, or Excel form (or type a few details). FillOnce saves them under People, encrypted so we can’t read them.
Open the next blank form
A standards-based web form in Chrome, or a fillable PDF, Word, or Excel sheet under Forms. FillOnce matches fields to your saved details.
Fill, review, submit yourself
Matched fields fill in. You check the page and submit. FillOnce never submits for you.
Built for real paperwork
Camp & school packets
Save last year’s form under People, fill this year’s blank packet.
Job applications
Save details from your résumé, then fill PDF or Word applications.
Rental & medical
Reuse income, insurance, and contacts without retyping every portal.

Your details stay yours
Browser autofill often syncs answers to a vendor. Password managers mainly cover logins and cards. FillOnce helps with PDF, Word, Excel, CSV, and standards-based web forms from details you save under People, encrypted in your browser so we can't read them. See how we protect data.

Frequently asked questions
Is FillOnce HIPAA-compliant?
FillOnce is a personal tool, you control your own data, and once your one-time setup is done it's stored encrypted with your key, so we can't read it. We never send your data to third-party AI services. We're not a covered entity under HIPAA, but the encryption model is built with medical data in mind, and you stay the owner. See our Privacy Policy for the details.
Can I manage forms for multiple people (kids, aging parents)?
Yes. The Family plan supports up to 6 profiles, each with their own medical history, insurance, medication list, and emergency contacts. Pick the right person on each new patient packet.
What kind of patient intake forms does it support?
Standards-based web intake forms (with the Chrome extension) and PDF or Word intake packets (in the app). Many patient portals deliver forms in these formats; we don't guarantee a specific portal without testing it first.
Can I import data from a form I filled last visit?
Yes. In Forms, choose “I have a filled form to save from,” upload last visit's intake as PDF or Word, and FillOnce saves medications, allergies, surgeries, and family history under People. The next structured blank intake can reuse those details when fields match.
What about prescription numbers, insurance IDs, and SSNs?
Stored encrypted (AES-256). You decide when they get filled. FillOnce flags sensitive fields for explicit confirmation before insertion.
